If you want to refer your child or a child under your care to Coordinated Kidz we welcome your referral. There are different forms you may require using depending on your type of funding for services.

NDIS Funding

We can assist your child with their therapy and access funding through their NDIS plan. If you have an NDIS plan and would like to use your funding please fill out the form below.

Medicare Funding

Enhanced Primary Care (EPC) plans can only be completed by a GP and the child must be present at each session.

Private Sessions

People who have private health insurance may be able to claim a rebate percentage or the full fee of an exercise physiology session depending on their insurance fund entitlements.
